- Signed Data Privacy and Non-Disclosure Forms
- SEC Certificate of Incorporation or DTI Registration
- BIR Certificate of Registration (BIR Form 2303)
- Two (2) Valid IDs of Licensed Broker (updated PRC ID & 1 Gov't. issued ID) and that of your Representative/Coordinator
- Company Profile
- Business Permit
- ITR (BIR Form 1702) – Previous Year
- Tax Clearance (BIR Form 17.14B)
- Blank Copy of Original Sales Invoice & Official Receipt with BIR Permit No. & TIN No.
- Latest photo of Licensed Broker and Representative/Coordinator
- Sworn Declaration
